Controlled production activation

Production activation is refused while any mandatory gate is unresolved — fail-closed.

Managed review

Recurring governed reviews begin; renewal is evidenced by signed reports.

Fail-closed go-live gates

Ten mandatory gates must be satisfied before production: live Entra login, SharePoint and ServiceNow validation (where in scope), HA / failover validation, container & OS image scan, external penetration test (or accepted risk), an approved telecom-SME golden set, a passed customer assurance run, a recorded restore drill, and go-live approval.

A gate counts as satisfied only when it is passed, not-required, or an accepted risk covered by an approved, non-expired production exception — and exceptions require two-person approval and always expire. Risk acceptance decays; it never becomes permanent.
